About the RoleWe are seeking an experienced Registered Nurse (RN) Case Manager / Care Coordinator for a 13-week contract in Chico, California. The RN will coordinate care and services for assigned patients throughout an episode of illness or treatment while collaborating with patients, families, physicians, payors, and interdisciplinary healthcare teams.
The role focuses on care coordination, utilization management, discharge planning, resource utilization, and continuity of care. The Care Coordinator may also be assigned to support patients in the Emergency Department.
ResponsibilitiesAssess, plan, coordinate, implement, and evaluate patient care services throughout the episode of care.
Collaborate with patients, families, physicians, interdisciplinary teams, and payors to facilitate appropriate and cost-effective care.
Coordinate discharge planning, transfers, community resources, and alternate levels of care to support safe transitions.
Review utilization and quality cases, including admission criteria, observation stays, readmissions, Medicare requirements, and diagnosis-related criteria.
Advocate for patients and families while maintaining communication among providers and monitoring progress toward clinical and resource outcomes.
Minimum 1 Year of Recent Case Management Experience in an Acute Care Setting Required.
Current, Active California Registered Nurse License Required.
Current BLS/CPR Certification Required.
Strong Knowledge of Care Coordination, Utilization Management, Discharge Planning, and Community Resources Required.
Ability to Work 8-Hour Day Shifts Across Weekdays and Weekends and Collaborate Effectively With Multidisciplinary Healthcare Teams Required.
