Job Description

Professional Case Management is seeking a dedicated Physical Therapist to provide in-home rehabilitation services to clients with complex medical needs, including former energy workers and their families. In this role, you will travel to clients’ homes to perform comprehensive physical therapy evaluations, develop individualized treatment plans, and deliver evidence-based interventions focused on improving mobility, function, pain management, and overall quality of life. You will collaborate closely with nurses, physicians, case managers, and caregivers to coordinate care, adjust treatment goals, and ensure safe, effective transitions in the home setting. Daily responsibilities include assessing strength, range of motion, balance, and functional abilities; instructing clients and families in therapeutic exercises, transfer techniques, and use of assistive devices; documenting care in accordance with regulatory and company standards; and monitoring progress to update plans of care. You will also educate clients and families on prevention of injury, energy conservation, and safe home environments. As part of a compassionate and collaborative team, you will participate in interdisciplinary case conferences, contribute to care planning, and support a culture of clinical excellence.
