Job Description

Community Connections, Inc. (CCI) is seeking to employ a full-time or part-time Certified Speech Language Pathologist (SLP) to provide services for children birth to 3 in Early Intervention (EI) and/or to participate as part of an EI evaluation team. Services are provided in homes and community settings in-person and at times via telehealth. EI promotes a coaching model (empowering parents and teachers to implement strategies between visits). Qualified applicants must delight in working with young children and enjoy being part of a collaborative team supporting families. Ability to provide services in Spanish is ideal but not required. Intimate knowledge of Indigenous or Hispanic cultures also ideal but not required.

FLSA: Hourly/nonexempt Part-Time or Full-Time 

Location: Hybrid Position based out of Durango or Pagosa Springs- not fully remote

Hourly Rate: $38.85 to $43.25 depending upon experience

Job Description : Early Intervention Therapists serve Community Connections as full-time or part-time employees working with children and families through the Part C/Early Intervention Program. Early Intervention services are provided via a coaching model to foster family involvement and empowerment. Schedules are flexible and are generally arranged with families and coordinated with the Early Intervention team as needed. Visits can be both in-person and at times via telehealth. Participation in Colorado Part C provider training is required. Must provide own transportation and be willing to travel around local communities to families’ homes, childcare settings and other community locations where children and their families are.

Responsibilities:

  • Become a Colorado Medicaid provider.
  • Maintain current and up-to-date licensing, certifications & endorsements in the respective discipline, and enter & maintain this information at least annually in the Office of Early Intervention Provider Database.
  • Participate in public & private insurance billing (including using CCI's EHR system) as requested and appropriate.
  • Provide required documentation to CCI's Early Intervention program to allow CCI to complete billing in a timely and accurate manner.
  • Provide clear case notes for use by families and to fulfill state requirements.
  • Assist in developing Individual Family Support Plans (IFSP).
  • Implement assigned strategies and action plans from the IFSP using a coaching approach with families/caregivers.
  • Work with the EI Service Coordinator to support each family to identify their priorities and gather therapeutic resources for the family.
  • Participate in trans-disciplinary teams and team meetings.
  • Attend IFSP meetings and reviews.
  • Provide ongoing assessment with family input, and participate in annual evaluations with an approved instrument as part of a professional team.
  • Participate in reporting child outcomes in accordance with State and CCI requirements.
  • Stay current in trends in Early Intervention, Best Practices and Evidence-Based Practices associated with the specific discipline.
  • Respond to families’ needs in a professional and courteous manner, building relationships with families and children that will become the basis for all interventions.
  • Communicate problems or concerns to the Service Coordinator or Early Intervention Program Director immediately.
  • Fulfill other duties requested by the Early Intervention Program Director to ensure compliance with the State and Part C Federal Assurances.

Qualifications: Must hold a B.A or Master’s in the appropriate professional field (Occupational Therapy, Physical Therapy, Speech and Language Pathology, Early Childhood Special Education, Social Work, etc.) and corresponding licensure for the State of Colorado. Experience working with children birth through three strongly preferred.

While performing the duties of this job, the employee is regularly required to be stationary, move indoors and within the community, position self, lift up to 50 lbs. and transfer, use objects, communicate verbally, in writing, and via in-person, computer and phone, and observe. The employee will be required to attend meetings, training, and other community activities within the region of Southwest Colorado, with the possibility of occasional travel within Colorado or out of state to training or conferences.
